Misc warnings from Ruby 1.9

There's a handful of missing initializers which produce thousands and thousands of warnings from Ruby 1.9 in -w mode. There are a couple of other warnings.

The attached patch:

  • Fixes a number of mismatched def/end pairs (caused mostly by a big chunk of a file being one space overindented)
  • Adds a number of initializers to some classes -- for instance variables referred to by mixed-in modules
  • Removes a few attr_foo declarations which are overridden by explicit definitions.

Unfortunately, some of the missing initializers are in the middle of the big hunk of whitespace diffs.

    I missed one, BoundingBox also needs its stretchy height initialized if you want to use boxes without a fixed height.
